Insurance Policy

Dr. Falcone is committed to spending a generous amount of time with each patient in order to get to know them, hear their medical story, prescribe the proper treatment, conduct the necessary teaching and answer any questions the patient may have. As a result, our practice must uphold the following policy:

• Dr. Falcone is an "out of network" physician for all private insurance plans. Full payment is expected at the time of the visit. Patients can be provided a receipt to submit to their insurance company or our office can submit a claim to the  insurance on the patient’s behalf, and insurance may reimburse the amount (if any) that they cover pursuant to the individual insurance policy.  

• Dr. Falcone has "opted out" of Medicare. As such, Medicare guidelines forbid our office from submitting any claims to Medicare either directly or through secondary insurance. 

• Patients will be asked to sign a "private contract" indicating that they are responsible for full payment at the time of the visit and that the visit fee will not be reimbursed by Medicare. 

• All laboratory tests sent out of the office and radiology studies requested by Dr. Falcone will continue to be covered at the discretion of Medicare or of your private insurance company. 

• Dr. Falcone is not responsible for the amount of reimbursement patients receive from their insurance company.
